Job Summary

• This position is responsible in a clinical care advisory role to assess members for wellness education and disease management;
• introduce company website tools to members to encourage personal health management; assist in program development / projects;

This is a remote position doing telephonic outreach

JOB REQUIREMENTS :

• Current, unrestricted license in the state of operations in one of the following : Registered Nurse (RN), or Licensed Professional Counselor (LPC), Registered Dietician (RD) or Licensed Master Social Worker (LMSW), Licensed Clinical Social Worker (LCSW), or Exercise Physiologist
• 3 years clinical experience.
• 1 year case management or condition management experience in a managed care / health insurance company
• Patient education experience.
• Computer / system software experience.

• Registered Dietitian
• CSR Certification
• Renal Experience
• Bilingual English / Spanish
